Having a big waist may raise women's death rates, even in women who aren't overweight. The results of a recent study of 44,600 female nurses who were enrolled in a long-term health study showed that large waists were worse for you than being overweight. Being in the normal weight range was less important than having a waist less than 34.6 inches and a waist-to-hip ratio of less than 0.88 . To calculate your waist-to-hip ratio, divide your waist measurement by your hip measurement. "Although maintaining a healthy weight should continue to be a cornerstone in the prevention of chronic diseases and premature death, it is equally important to maintain a healthy waist size and prevent abdominal obesity," the researchers reported in the April 1 edition of Circulation. When the nurses were 40 to 65 years old, they measured their waists and hips for the study. At the time, none had had heart disease or cancer. Every two years, they updated their health and lifestyle records for the study, including their physical activity, smoking, alcohol use, and menopausal status. The nurses were followed for 16 years. During that time, a total of 3,507 of the nurses died, including 751 who died of heart disease and 1,748 who died of cancer. Regardless of other factors, including BMI (body mass index, which relates height to weight), women with larger waists and greater waist-to-hip ratios had higher death rates from all causes, including heart disease and cancer, which are the top two killers of U.S. women. For example, among women of normal weight, those with a waist larger than 34.6 inches were three times as likely to die of heart disease, compared to women with smaller waists. Large hips weren't a problem, if the waist wasn't also large. In fact, having large hips and a small waist was associated with lower risk of death from heart disease. Simply measuring the waist is all that's needed. The researchers found that waist-to-hip ratio wasn't a better predictor of death rates and was more effort. Cuilin Zhang, MD, PhD, of the National Institute of Child Health and Human Development used the definitions for abdominal obesity recommended by the American Heart Association and U.S. Department of Agriculture. Those cutoffs are waist circumference of 34.6 inches for women and 40 inches for men. The study doesn't necessarily prove that abdominal fat is lethal. Uterine fibroids are benign tumors found in the womb or uterus. They are composed of dense fibrous tissue and can cause a variety of symptoms including excessive menstrual bleeding, pain, abdominal swelling and infertility. Fortunately, there is a wide range of effective treatments for fibroids. The medical term for uterine fibroids is leiomyoma. A rare cancer of the uterus is called a leiomyosarcoma, but having fibroids does not lead to this tumor or increase the risk of it. However, fibroids can have serious impact on a woman's health. No one knows for sure what causes fibroids but there are risk factors that make them more common, obesity being one of them. They are also more common in women in their 30's and 40's although they often shrink after menopause. African American women are more likely to have fibroids and so are women with a family history of the condition. While the causes of fibroids are not known, the risk can be decreased by avoiding obesity and the consumption of a healthy diet rich in vegetables. From 20% to 80% of women will develop fibroids in a lifetime. It is known that diet can have an influence on the development of fibroids with the consumption of lots of vegetables and fruits being protective while large quantities of red meat seem to encourage fibroid growth. Some fibroids cause no symptoms at all, even when sizeable. Common symptoms of fibroids are heavy menstrual periods, heaviness in the lower abdomen and pelvis, urinary symptoms and painful sexual intercourse. Fibroids can complicate pregnancy and greatly increase the chances of requiring a cesarean delivery. Fibroids are classified by where in the uterine wall they occur. Submucosal fibroids grow into the uterine cavity while intramural fibroids are contained inside the muscle wall and subserosal fibroids grow on the outside of the womb. Pedunculated fibroids grow on a thin stalk and look something like a mushroom. There are many different means of treating fibroids when they require treatment at all. Asymptomatic fibroids only require watching for problems and are common. When symptoms cause a great deal of pain or anemia from heavy bleeding, there are surgical and nonsurgical options. Size and location of the fibroids may dictate the types of therapy available. For women who do not wish to preserve childbearing, hysterectomy or removal of the uterus is a direct solution. It is also often possible to remove just the troublesome fibroids by a procedure called myomectomy. Conception may be possible after that option. Another option is uterine lining ablation which is done with a variety of techniques. After ablation, conception is not possible. Medical control of fibroids may be as simple as the use of NSAIDs or acetaminophen for discomfort or may use hormones and anti-hormonal drugs to control symptoms. Low dose birth control pills may work for some women while others require hormone suppressing drugs such as Lupron or mifespristone. The problem with these drugs is that the fibroids may grow again when the medication stops. Women with fibroids may not even know they are present or may have significant symptoms, but modern treatments make this diagnosis one that should not be terrifying. Diagnosis can usually be made by imaging with X-ray, ultrasound or MRI but occasionally is done by inserting a special scope into the uterus through the vagina (hysteroscopy) or through the abdominal wall (laparoscopy.) How to Build the Ultimate Circuit Training Workout for Rapid Fat Loss
In the past, the cornerstone of most resistance training workouts has been performing exercises in a straight set format. Essentially, this means you perform a certain number of reps for a given exercise or as many reps as possible for a given exercise and then you rest anywhere from 30 seconds to five minutes based on your training goals before repeating this set a certain number of times based on your goals. For more of a fat loss and lean muscle gain emphasis shorter rest periods are ideal. For more of a strength or power emphasis, longer rest periods are prescribed. So, what's the problem with this format? Well, though straight sets are extremely easy to comprehend for people new to fitness, they are also an extremely inefficient way to order your exercises. In most commercial gyms you'll typically see someone perform three sets of 10 reps for let's say the bench press. They pump out 10 reps go to the water fountain, talk with some buddies, watch a couple of highlights on ESPN, and then gingerly walk back to the bench area to hit up their second set. In general, most people, guys in particular, tend to take at about 3-5 minutes between sets. In the case of three sets of 10 on the bench that means it would take up to 15 minutes to complete only one movement pattern (a horizontal push). That means you'd need at least in hour to perform four different exercises! A much more effective and time-efficient approach to ordering your exercises is utilizing the alternating set format. Here you'll perform one exercise, rest for a short period of time, then perform another non-competing exercise, rest for a short period of time, and so forth. Alternating sets allow you to work different areas of your body when you would otherwise be resting with the straight set format. Plus, by working another area of your body with a non-competing exercise you allow your body to recover from the previous exercise(s). The result is improved training economy and density: more work accomplished in less time, the cornerstone of any sound fat loss program. There are several ways to perform alternating sets outlined below: 1.) Supersets: Alternate between two different non-competing exercises (e.g. upper body and lower body such as push-ups and lunges) 2.) Trisets: Alternate between three different exercises (e.g. push, pull, and lower body such as push-ups, rows, and lunges) 3.) Circuits: Alternate between four or more different exercises Though supersets and trisets are excellent alternating set options, I believe circuit training is by the far the best option time and time again. To demonstrate why, let's examine my favorite circuit training template using timed set intervals: The 50-10 Five Exercise Circuit: You will alternate between 50 s of work and 10 s of rest for all five exercises in the following 5-minute circuit: Exercise#1- Squats Exercise#2- Dips Exercise#3- Single-Leg Hip Extensions Exercise#4- Pull-ups Exercise#5- Leg Raises Perform this circuit up to four times for a 20-minute total body fat burning workout. Basically, in the same 15 minutes that it took to get in three sets on the bench you could have gotten in three sets of five different exercise for a staggering total of 15 work sets! Plus, the intensity on each exercise will be just as high as in the straight set format because in this five exercise circuit you will have full recovery with over four minutes before you return to any given exercise (just as you did with the straight set format described earlier). To recap, the key to creating the optimal hormonal environment for fat loss is to perform each exercise with maximal intensity while separated by brief rest periods in order to accumulate a high volume of total body work in the shortest amount of time possible. Circuit training provides for the best of both worlds and is thus simply unmatched for simultaneously maximizing fat loss and lean muscle gain. Many people have been asking us recently about whether we all have a "Brain Age" and how we can reduce our "brain ages". This concept is a myth, fueled by the (very fun) Nintendo game and a recent PBS campaign promoting a program produced by Posit Science. The concept of having a "brain age" is, itself, profoundly unscientific, despite the radio ads for the PBS program titled Brain Fitness Program, where listeners of all ages get the impression (as many friends and colleagues have reported) that, should they buy the Posit Science Brain Fitness Program, they can expect their brains "rejuvenated" by 10 years. This, I hear often, must be true, coming from PBS. Unfortunately, it isn't. And it isn't because the claim is founded on the same faulty premise of having a "brain age". What is going on? First, the good news. Today we know today that the brain retains lifelong plasticity (the ability to change itself through experience). Aging does not mean automatic decline. Furthermore, we know that a variety of lifestyle factors, including physical and mental exercise, can influence how our mental abilities evolve as we age. We can delay or slow down age-related decline. Not only that, we can improve our abilities, and a number of computer-based programs have shown how they can help specific groups of people train and enhance specific cognitive skills. Now, what is important to recognize is that there is not one overall "brain age". We can view our brain functions or cognitive abilities as a variety of skills, some more perception-related, some more memory-related, some more language-related, some more visual, some more abstract-thinking and planning oriented. All science-based brain fitness products in the market today target specific cognitive skills. The research that has been published shows how specific brain functions can be improved. But there is no general "brain age" that can be measured or trained in a meaningful way. Let's analyze the PBS Posit Science-related message: you can rejuvenate your brain by 10 years. What would this mean, were it to be true? perhaps that ALL cognitive abilities would go back to where they were 10 years before. and that this would happen for individuals of all ages: in our 30s, 40s, 50s, 60s, 70s and so on. It would also mean that, given that rejuvenated "brain age", our risk of developing Alzheimer's symptoms would be adjusted to reflect our "new" brain age. And that the evolution of our cognitive abilities over the rest of our lives would reflect our new-found "brain age". Has this been shown? Unfortunately, not. The "10 years" claim seems based on one published study, and several unpublished ones, where individuals with an average age of around 70 years take on a very intense auditory processing training program that enables them to improve related auditory cognitive skills by a significant percentage. Whereby, on average, and on those specific skills, they reach a level comparable to people 60 years old. But this doesn't say anything about other cognitive skills. Or Alzheimer's related risks. Or the cognitive trajectories that will follow. Just think about this: if, by attending an intensive tennis camp, you were able to serve at a level comparable to people 10 years younger than your age...would you say that your body is now 10 years younger? Probably not. You'd say that now you play tennis better. Which is a significant benefit in itself if that's what you are after. Recent studies have shown a tremendous variability in how well people age and how, to a large extent, our actions influence our rate of brain improvement and/or decline. The earlier we begin the better. And it is never too late. What can we do to maintain our brain? Focus on four pillars of brain health: physical exercise, a balanced diet, stress management, and brain exercise. Stress management is important since stress has been shown to actually kill neurons and reduce the rate of creation of new ones. Brain exercises range from low-tech (i.e. meditation, mastering new complex skills, lifelong learning and engagement) to high-tech (i.e. using the growing number of brain fitness software programs). In summary, the great news is that there are more tools available than ever before to assess and train a variety of cognitive skills, in what is still today a very small, but growing market. Nintendo, Posit Science, and others are offering valuable products and services. The bad news (is this really news?) is that we shouldn't be expecting magic pills and that "brain age" is a fiction. In case you wonder...I do have and enjoy my copy Nintendo Brain Age, and appreciate it as a stimulating game. You, or a loved one, may need to be hospitalized in the near future. This can be a time of confusion and extreme stress, and not only for the patient. But with some planning and recordkeeping prior to, during, and after release from the hospital, you can make the whole process easier on everyone. There will be questions that you need to ask so that you can effectively participate in decisions about your health and treatment. Also, consider keeping health care planning information with your general financial and investment documents. This will help you to have a clearer and more complete picture of your overall plan for the future. 1. Do I need an advocate? By using this article as a guide, you can list questions you need to ask, when to ask them, what information you need to record, and note if follow-up information is required. If you are the patient, have someone act as your advocate or at least have someone who will keep track of the information that needs to be gathered. Oftentimes, the patient is not in the condition to ask questions or record answers, and may not have the energy to be an advocate. Having an extra set of eyes and ears available, and someone who can ask questions on your behalf, can often make a big difference in the course of the patient's treatment. 2. What will I bring to the hospital? When initially planning for a hospital stay, consider what items you want to bring to the hospital to make your time there more comfortable. You may want to bring things from home, such as books, slippers, and a robe. You may want to bring a 2- or 3-day supply of your prescribed medications so that there is no lapse in medication while you are waiting for re-fills at the hospital. You also may want an eye mask so you can sleep during the day, and ear plugs for blocking out the extraneous noises that are ubiquitous in hospitals. 3. What information do I need to record? You will want to note the names of the doctors that have treated you in the hospital; tests that were performed and with what results; the diagnosis, in medical and layperson's terms; the prognosis for your condition or disease; any new medicines that are prescribed; possible side effects of proposed treatment; what you need to do in order to be able to leave the hospital; and information about any appropriate clinical trials. 4. How do I keep track of medicines? It is especially important to keep track of medicines prescribed before, during, and upon release from the hospital. Keep an ongoing list of all of your medicines, including when the medicines are first prescribed, when dosages are adjusted, and when medicines are discontinued. An accurate list will help protect you from over-medicating, under-medicating, or having ineffective combinations of medications. Have your doctor review your medication list before you leave the hospital to be sure that it is accurate. 5. What is a discharge meeting? Prior to your release from the hospital, there will be a discharge meeting. The discharge planner coordinates ordering equipment, home health services, outpatient therapy, and many other services. The planner will make sure that the doctor has issued prescription orders for all services you need. It is a good idea to attend the discharge meeting (or have someone attend on your behalf) so that your needs may be more fully addressed. 6. Will I go to another facility when I am released from the hospital? Also at the discharge meeting, it will be decided if you will go home, or to some other facility. You may be placed in a rehabilitation hospital, a skilled nursing facility, an assisted living residence, or home with help from a caretaker, family member, or friend. 7. What about accessibility and mobility? Issues of your mobility can be assessed at the hospital, especially if you request it. Many people leaving the hospital will have specialized equipment for their recovery period. Beyond a wheelchair and walker, there are other items of "durable medical equipment" that help with accessibility and mobility, such as grab bars, raised toilet seats, and railings for the bathroom. If you are going to a rehabilitation hospital, skilled nursing facility, or assisted living facility, many of these items are provided for you while you are there. 8. How well do I have to be before I am discharged? Also, prior to your release from the hospital you need to be able to accomplish certain tasks. These can include getting out of bed by yourself; being able to walk to the bathroom; having a bowel movement; and/or being hydrated to a certain level. 9. What physical or other restrictions will I have? When you are released, it will be important to understand what restrictions you will have relating to food, drink, and physical activities. For example, will you be able to take a shower, drive, drink alcohol, and/or eat normally? This information will help your caregivers take better care of you and help to speed your recovery. It is also critical to confirm which medications you are to take once you are released. Double-check to see what medications have been added or removed during your hospital stay. 10. Will I go to a rehabilitation hospital or a skilled nursing facility? If you are in need of intensive therapy before you can return to your home, you may be released to a rehabilitation hospital. A rehab hospital will often provide several hours a day of therapy for the patient. This is a place for patients that are too "well" for the hospital but not yet strong enough to return home. Another option for care between the hospital and the home setting is a skilled nursing facility, which will usually offer some rehabilitation services. At times, you will need more help than you can get at home, and you will be released to an assisted living facility. 11. What do I need to consider if I am to return home? Additional details are needed if you are going from the hospital to your home. Will your necessary equipment be there already? What parts of the home will or will not be accessible to you? What needs to be done to make your home more comfortable and accessible for you? If you ask questions in advance, adjustments can be made so that your return home goes more smoothly. While a hospital stay is never looked forward to, with some planning, asking the right questions, and recording information, the stay may be less stressful. What Every Athlete needs to apply to their training in order To enhance their sport specific speed I've got the need...The NEED FOR SPEED!!! And you should too. Every athlete needs to realize that they should always be searching for new ways to improve their speed. That's because in sport speed kills. You can never be fast enough. However, it's not just how fast you can run from point A to point B. You have to be able to be as fast as possible while moving in various directions, starting and stopping constantly. After all, what sport do you know of where change of direction doesn't happen? Not Many. So let's cut right to the chase... Here are the top ways to enhance your sport specific speed for any sport: 1.) Be specific - to the demands of what type of speed you need in order to excel at your respective sport. For example, the type of speed needed for a football defensive lineman is totally different than the speed needed for a midfielder in lacrosse, or a forward in basketball. So your speed training must reflect the demands of your specific sport. 2.) You won't get faster by simply running around a track - A major misconception to speed training has been to just run like a maniac around a track and expect to get faster. The problem here is that you may need other methods of training such as stretching, strength training, core training, balance and/or stability training in order for you to get faster. Maybe it's a lack of flexibility that's slowing you down 3.) Stop training like a body builder - Instead of training on that hamstring curl machine, try some multi-muscle movements like Olympic lifts (snatch / Cleans), squats, and dead-lifts. Not only are these exercises better than any kind of isolation based exercise (hamstring curls and leg extensions), they actually strengthen your leg muscles (and entire body) and train them to work together as a unit. While you're at it incorporate some single leg exercises such as lunges, and single leg deadlifts. By doing so you will further mimic your body's running actions. Find your weak links, and correct muscle imbalances - You are only as strong as your weakest link, and muscle imbalances will always lead to weak links in your body. Breaking Through your Weight Loss and Body Transformation Plateaus
The biggest challenge when starting any Nutrition & Fitness program is learning how to Break Through Plateaus. Plateaus are a natural part of the process, so you will experience them, the key is to understand & implement the necessary tools to overcome them when you do. Through my years of coaching I have heard 4 main Questions and Statements from Clients: 1. "When do you think I will Plateau?" 2. " I can always get an initial result, then my body stops progressing." 3. " I cannot seem to lose these last 10 Pounds." 4. " I cannot seem to put on any muscle." Most people will come upon a Plateau like those just described. I would like to share the 5 tools that will enhance your education and provide you with the necessary strategies to Break Through these Plateaus. The 5 tools consist of: Tool 1: Accept that Plateaus will occur Plateaus happen in all aspects of life, nothing can progress at the same pace forever. Sometimes, they must slow down. For example, take a look at the growth of a child, the process of building a business, or the deepening of a relationship... They all possess moments of progression followed by moments of maintenance. Accepting that plateaus will happen is part of the process. I will explain below how to break through them. Tool 2: Understand Your "Set Point" Your body has an Internal Mechanism called a "Set Point" - similar to the temperature on a Thermostat. Your genetics, gender and the choices you make determine what that set point is. When anyone starting a nutrition program initially gets a result, the most common reason is that their body is above their "Set Point". Imagine setting your home's thermostat to 80 Degrees. If the temperature is at 85, the air conditioning will switch on. However, once the room reaches 80, the cooling will stop. Your body works in a similar fashion. Once you reach your Set Point (like the temperature on a thermostat), your body will also stop progressing, unless you have the proper foundation. The key to Breaking through your Plateaus is to Reprogram your "Set Point" (adjust your Thermostat). Tool 3: Create a Foundation Your foundation is the key to your success. The only way your body will reprogram its set point is if it achieves Homeostasis (internal Balance). When people "Diet", they cut & restrict food intake, leading to an initial result, unfortunately dieting does not create Homeostasis in your body, so the result is followed immediately by an unbreakable Plateau. This leads to the "YO-YO" dieting syndrome. The same issue is also relevant to individuals who're looking to put on Muscle. "Hard Gainers", most times will attempt to gain muscle mass by overeating, but they come up short as well - they haven't achieved Homeostasis. The key for Weight & Body Fat loss as well as Increasing Muscle Mass is to Stabilize your blood sugar levels; this is the only way to create Homeostasis and provide the body with the proper environment in which to reprogram its set point. When the body is in balance (has Stable Blood Sugar) it has all the proper nutrients and an ideal environment to achieve any goal. Once your blood sugar is stable, what else is needed for a solid foundation? The answer is a Health Structure. There are six main components which when looked after, will provide optimal balance for the body: Sleep, Nutrition, Exercise, Supplements, Water intake, and the separate component which combines aspects from all of the others - Stress Management. Please remember, you WILL face Plateaus, so now you've got to focus on staying ahead of the curve. I suggest enhancing your foundation (Health Structure) by improving the quality of each component of your Health Structure - becoming more efficient. Breaking through plateaus is not necessarily about doing more; it's about doing better, becoming more efficient and diversifying what you do. Tool 5: Stay Consistent Consistency is the X factor that will provide you with the ability to accomplish all of your Health and Fitness goals. As long as Tools 1-4 are being implemented, you will continue to make progress. The closer you come to your goals, the more resistant and slower the process will be. This is a natural defense mechanism of the body. However, as long as you stay the course, and implement these strategies, you will Break Through all of your Plateaus. In Conclusion... Many people, both men and women, struggle with controlling weight and maintaining good health. 12 Questions: From Hospitalization to Home
If you are ready to return home from the hospital to recuperate from treatment for a disease or injury, there are 12 questions you need to consider. You will want to address issues of accessibility, mobility, and the amount of assistance and outside care you will need. 1. Who decides when I can return home? While you are in the hospital, the medical staff considers your needs and decides to which setting you will be released. If you are able to recover without a lot of extra assistance, you are returned to your home. In situations where more care is needed, a patient may be moved to a rehabilitation hospital (where intensive therapy is available) or to a skilled nursing facility (where some therapy is available). 2. How do I know if the rooms in my home will be accessible to me? In planning for your release to your home, the home setting needs to be evaluated to determine which rooms are accessible and which are not. You may need for a therapist on staff to inspect your home for safety and suitability before you return there. For instance, the number of steps into the house is important to know and, also, if there is a second floor. 3. Will a wheelchair or a walker fit through the doors in my house? If you need a wheelchair, make sure that at least one side of your bed has enough room between the bed and the wall so you can park the wheelchair next to the bed for easier transfers between chair and bed. 5. Do I need to make changes to my bathroom? If the bathroom is not wheelchair accessible, you may need help from someone in order for you to walk from the doorway to the toilet, or you may need to rent a commode chair and place it in another area of the house until you are steadier on your feet. It may be that the bathroom is accessible, but you need a raised toilet seat and grab bars next to the toilet so you can get off the toilet safely. You may need a chair for inside your bathtub or shower. Find out if you will be able to reach the bathroom and kitchen sinks in the house or if you will need assistance. 6. Will I need a hospital bed for use at home? If your bedrooms are on the second floor, you may need to use a hospital bed on the first floor until such time that your mobility improves and you can handle the stairs. The fact that you can adjust the height of the hospital bed may make it more comfortable for you to get in and out of the bed. 7. How can I use my telephone and television? You will need to be able to reach the telephone, so you may need a longer cord and a lower table for the telephone. The television needs to be in a location where you can watch it, so it may need to be moved and you may want to find an easy to use (larger numbers and buttons) remote control. 8. What kind of activities can I continue to do? There are other issues having to do with mobility and accessibility that will depend on your particular lifestyle, so think of what a typical week or month would be like for you. Figure out who will be able to help you get to appointments, if necessary. If you attend religious services and you need help getting there, find a friend who will pick you up and bring you back. 9. What if I need extra help with bathing, for instance? Depending on your needs, you may have someone stay with you to provide help. A home health care aide may be available through home health services to come in and help with bathing, grooming, and dressing. Check to confirm to what extent these services are covered by your insurance. You may also receive physical, occupational and/or speech therapy through the home health agency. 10. Can I get someone to help me prepare meals? In addition, you may need help with chores, meal preparation, and shopping. You will need to find out who can help you with these tasks. In some areas, your county may have in-home help available if you meet the income requirements. The discharge planner at your hospital will know about resources available in your local community. Also, you may be eligible for Meals-on-Wheels for a period of time: confirm how many meals per day and for how many days you can receive this service. Please remember, Meals-on-Wheels is usually an out-of-pocket charge, meaning you pay for this service without reimbursement from your insurance company. 11. My finances are a mess. What can I do? You will need to take care of your financial issues and money management. If you are not able to handle the mail, paying bills, and banking, find someone who can help you. It may be a family member, a friend, an attorney, or other fiduciary that you choose. 12. Will I be able to keep my pets at home? If you have pets dogs and cats are especially good for promoting one's well-being and recovery be sure to have details available regarding your veterinarian, boarding facility, and who will pet-sit in an emergency or as a favor. Your favorite pets by your side may be the best part of being home. Be especially careful navigating around your house if you are using a wheelchair or walker and your pets tends to get underfoot this could cause an injury to you or to your beloved pets. Also, you need to store the pet food where you can reach it if your mobility is impaired. Remember, planning ahead will made your recovery go more smoothly and you will experience less frustration if you have planned for your particular needs and restrictions. It is generally agreed by many physicians that the primary reason for Hormone Replacement Therapy (HRT) is symptom relief from menopause, with less emphasis on using hormone therapy for disease prevention. It is important that the woman know all risks and benefits associated with HRT and reminded that the risk for breast cancer does increase naturally for all women as they age, as does the risk of heart disease and osteoporosis. "Women with a uterus who are currently taking estrogen plus progestin should have a serious talk with their doctors to see if they should continue it. If they are taking this hormone combination for short-term relief of symptoms, it may be reasonable to continue, since the benefits are likely to outweigh the risks. Longer term use or use for disease prevention must be re-evaluated, given the multiple adverse effects noted in Women's Health Initiative (WHI)." One physician associated with the WHI says, "always cautions patients about the potential for increased breast cancer risks." First, she rules out women who are not candidates for HRT - those with bleeding problems of an unknown cause, suspected breast cancer or history of breast cancer, history of endometrial cancer or certain cancers of the uterus, chronic liver disease such as cirrhosis or a history of blood clots. She further tells her patients who want to stop HRT that they can certainly quit anytime. First of all, with menopause, we're not treating a disease, and stopping HRT has no major consequences, except perhaps a return of the original menopausal symptoms. For both women who want to stop taking HRT and for women who choose not to start HRT, there are alternative therapies. For almost everyone, there are other treatment options. For instance, women they can reduce their risk of heart disease by stopping smoking and by keeping their weight, cholesterol levels and blood pressure under control. Prozac and some other antidepressants can relieve hot flashes. Prescription drugs such as Fosamax help protect against osteoporosis. Also, the drug Evista (raloxiphene HCI), prevents osteoporosis and further claims to lower total cholesterol and prevent breast cancer. However, because women on Evista may experience more hot flashes, it raises questions about how that might affect the brain. Research now suggests a link between hot flashes and Alzheimer's. Evista belongs to a class of drugs called SERMs, or Selective Estrogen Receptor Modulators. Hormone Replacement Therapy and Black Cohosh
Modern experience with Black Cohosh dates back to the mid-1950s. In Europe, doctors concerned with finding an alternative to hormone replacement therapy (HRT), which even then had recognized unwanted side effects, reported success surrounding the treatment of menopausal symptoms. In the early 1960s many medical reports (although not controlled clinical trials) involving over 1,400 women were published in Germany. Health care practitioners documented benefits in premenopausal and menopausal symptoms including reduction in hot flashes and improvement of "depressive moods." Furthering the advancements, five clinical studies since 1979 have compared Black Cohosh extracts with placebo and estrogen replacement in the treatment of menopausal symptoms. One study that was done in several clinics with information on 629 patients reported favorable results in more than three quarters of the participants after six to eight weeks of treatment. Improvements included relief of stereotypical problems: hot flashes, sweating, headache, dizziness, and rapid heartbeat. Some side effects that were not documented were reported in less than 10% of participants, but were not significant enough to stop taking. Black Cohosh was actually introduced into medicine by Native Americans, who placed a high value on it. American Indians boiled the roots in water and drank the beverage for a variety of conditions ranging from rheumatism, diseases of women, and the pain of sore throats. It's subsequently used, especially by the Indian medicine man, for all these conditions but mostly for so-called uterine difficulties (regularity of cycles). Scientific studies have shown that a methanol extract of black cohosh contains substances that bind to estrogen receptors of rat uteri. Cohosh extract also causes a selective reduction in luteinizing (luetinizing is a female hormone produced by the anterior lobe of the pituitary gland) hormone levels in rats. These results are generally universally interpreted to mean that this herb possesses some degree of estrogenic (stimulating and leveling) power. A 1991 study confirmed an LH secretion inhibitory effect in both ovariectomized rats and in 110 menopausal women, demonstrating that the extract selectively suppresses luteinizing hormone secretion in menopausal women. A recent Asian study reported positive effects of two Asian Cohosh species, on calcium and phosphate levels plus bone mineral density in rats. The findings concluded that these extracts have potential for the treatment of osteoporosis, particularly in menopausal women. It is recommended in Europe for various conditions, including symptoms associated with premenstrual syndrome (PMS), dysmenorrhea, and menopause. Reported activities include an estrogen-like action, binding to estrogen receptors, and suppression of luteinizing hormone. Occasional stomach pain or intestinal discomfort has been reported. In North America, it is thought that Black Cohosh balances estrogen by stabilizing it. In European herbalism it is thought to have an estrogenic action, which actively works to reduce progesterone and promote estrogen levels in the body. It is therefore used where there is a lack of estrogen and an excess of progesterone. In the musculoskeletal system it is used as an anti-inflammatory in arthritic conditions. Its sedative qualities have applications in other systems, for example in lowering blood pressure, in reducing spasm and tension, and in the respiratory system. Native Americans used the rhizome of this cohosh as a cure for rattlesnake bites (hence its common name, rattle root) and for menstrual and labor pain. The root was also chewed as a sedative and to alleviate depression. A tea made with the herb was sprinkled in rooms to prevent evil spirits from entering. In herbalism, the root is still used as a diuretic, a cough suppressant, and to reduce inflammation and rheumatic pain. Black Cohosh is native to Canada and the eastern states of the US, growing as far south as Florida. This herb prefers shady spots in woods and shrubby areas. The herb is now grown in Europe and can be found in the wild, having self-seeded from cultivated plants. It is grown from seed, and the root is harvested in autumn. Female applications - Native Americans have long used Black Cohosh for female problems, for which it was also known as "squawroot." It is used today for menstrual pain and problems where progesterone production is too high and for menopausal symptoms, especially hot flashes, debility, and depression. Inflammation - Black Cohosh is useful for inflammatory arthritis, especially when it is associated with menopause, and it is also an effective remedy for rheumatic problems, including rheumatoid arthritis. Sedative properties - the sedative action makes it valuable for treating a variety of conditions, including high blood pressure and tinnitus (ringing in the ears). It is also valuable for whooping cough and asthma. More and controlled studies are warranted on Black Cohosh.
